Supporting Seniors to Live Well, Stay Independent, and Feel Supported
At The Information Center, our focus has always been simple: helping seniors live safely, independently, and with dignity in the community they call home.
Today, we continue that commitment by supporting older adults and their families through some of life’s most important transitions — whether that means finding the right services, navigating long-term care options, or returning home from a nursing facility. We specialize in community-based supports, including help through Michigan’s Medicaid waiver programs, guidance for caregivers, and Community Transition Services (CTS) that assist individuals who want to move from a nursing home back into the community. Our team works alongside seniors to understand their goals, explain their options, and connect them to the supports that best fit their needs.
The systems that serve seniors can feel complicated. Our role is to make them clearer, more personal, and easier to navigate — so no one has to figure it out alone. The Information Center provides the “personal touch” our seniors deserve. As the needs of older adults continue to change, The Information Center remains focused on what matters most: trusted guidance, practical support, and a strong belief that people do better when they are supported in their own homes and communities. If you or someone you love needs help understanding available services or planning next steps, we’re here to help.
Our care management department of nurses and social workers is here for you, and our Community Resource Navigators are just a call away, ready to assist you, your family, and loved ones in finding the free community resources they need.
